Walk-in Applicant Checklist

When you come to the Embassy, make sure that you have all of the following materials. If anything is missing, we will not be able to accept your application.

Print this sheet, and place a check by each item once you have confirmed that you have it.

♦  Walk-in Applicant Checklist (this document)

♦  Current passport for all applicants

♦  Cashier’s Order for S$160 per applicant from a local bank, made payable to   "American Embassy, Singapore"

♦  One completed and signed DS-156 application form for each applicant. You must use the Electronic Visa Application Form (EVAF), which will print out as 3 pages, the last page being a barcode.  All applicable fields must be filled in on a computer and printed on the form, not handwritten.  Handwritten DS-156 Forms Will Not Be Accepted and You Will Have to Return Another Day to Apply.

    ♦  Citizens of Vietnam must bring two signed copies of the DS-156

    ♦  One two-inch by two-inch photo for each applicant, attached to the DS-156

    ♦  Form DS-157 (only required for male applicants aged 16-45)

    ♦  Any additional documents required for your particular visa category

    ♦  Sufficient cash (U.S. or Singapore currency only - no checks, credit cards or  NETS) for any visa issuance fees, if necessary.  Issuance fees are in addition to the S$160 application fees.  Every applicant must pay the application fee by Cashier's Order, but issuance fees must be paid in cash.  Please check here to determine if you have any issuance fees.

The following are not required, but will assist the Visa Officer in adjudicating your application:

  • Prior passports for the past ten years
  • Any supplemental documents you wish to show the Visa Officer
