Walk-in Applicant Checklist
When you come to the Embassy, make sure that you have all of the following materials. If anything is missing, we will not be able to accept your application.
Print this sheet, and place a check by each item once you have confirmed that you have it.

♦ Current passport for all applicants
♦ Cashier’s Order for S$160 per applicant from a local bank, made payable to "American Embassy, Singapore"

All applicable fields must be filled in on a computer and printed on the form, not handwritten. Handwritten DS-156 Forms Will Not Be Accepted and You Will Have to Return Another Day to Apply.

♦ One two-inch by two-inch photo for each applicant, attached to the DS-156
