Replacing a Mutilated or Damaged Passport

If your passport has been mutilated or damaged so that it is no longer in good enough condition to be used to travel, you may request that it be replaced. Bring the damaged passport along with the following documents to the ACS section at the Embassy:

  • A completed form DS-82 (Print Form DS-82) for adults who have previously held a 10-year passport, or DS-11 for children who have never held a 10-year passport (Print Form DS-11); do not sign form DS-11 as you will be asked to sign in the presence of the Passport official. The form may be obtained at the ACS office or a U.S. passport agency. Be sure to include your social security number on the application form, as we cannot process your application without it.
  • If the damaged passport belongs to a child under the age of 14, both parents must appear, together with the child, unless one parent has provided a notarized Letter of Consent (see Renewing a 5-Year passport for a Child for additional information about this rule).
  • An affidavit explaining under penalty of perjury the circumstances under which the damage occurred. A blank affidavit form may be obtained at the ACS office. Do not sign the affidavit as you will be asked to take an oath and sign in the presence of the Passport official.
  • Two recent 2" x 2" (50mm X 50mm) passport photos. Please review the Passport Photograph Requirements.
  • Proof of U.S. citizenship. Acceptable documents proving U.S. citizenship are: a prior U.S. passport (other than the emergency passport), a U.S.-issued birth certificate, a Consular Report of Birth Abroad, a Certificate of Citizenship, or a Certificate of Naturalization. Other documents, such as U.S. Driver's Licenses, Military ID cards, Credit Cards and the like are NOT evidence of citizenship.
  • Fee of US$100 for adults or US$85 for children.

If you are replacing a damaged passport, you are required to appear in person.
