Appointment System for Passport Applications for Minors (children under 16 years of age)
Passport applications for minors (children under 16 years of age) will no longer be accepted on a walk-in basis from April 20, 2009. To apply for a passport for a minor, you must make an online appointment prior to coming to the Embassy. This online appointment system will minimize the time the child and both parents must spend in the Embassy.
Review the application information on this webpage carefully and gather the required supporting documents. Once you are prepared to submit a complete application, with all supporting documents and forms, make your appointment from the link below. You will need to select the button labeled, “Passport services other than adding pages,” and then follow the instructions to reserve an appointment for a passport for minor renewal. Also let us know if you will require multiple services. For example, if you have more than one child renewing a passport or if you are submitting an adult passport for renewal you can write this information in the box where the names of non-applicants are to be given.
All services other than passport applications for minors and Reports of Births Abroad will continue to be available on a walk-in basis. Our appointment portal, however, currently shows the whole range of passport and documentary services. At this time if a service other than Report of Birth of Abroad or “Passport services other than adding pages” is selected, you will be guided to a nonfunctional webpage and be forced to use your “back” button to navigate back to the “Choose Service(s)” page.

Requests for emergency/expedited appointments
First, try scheduling an appointment. Please do not request an emergency appointment if we already have available appointment time slots for the next business day.
The U.S. Embassy in Singapore will accommodate requests for emergency appointments for humanitarian reasons or, on a space available basis, for truly urgent travel. Failure to plan adequately ahead for business or personal travel does not constitute an emergency.
If you have an emergency and have attempted to schedule an appointment but have found there were no more slots available, please send us an e-mail to SingaporeACS@state.gov to request an emergency appointment. Please type "Emergency Appointment Request" in the e-mail's subject line. Please include the following:
- Name and Contact Information; and
- A description of the emergency; and
- The type of service required; and
- The date and time of the appointment you have already scheduled.
We will reply to you as soon as we can. We will not be able to accommodate all requests. Therefore, it is urgent that you plan well in advance for your trip to the U.S. Embassy.
New Passport Application Requirements for
all Minors under 16 years-old
Starting February 1, 2008, new regulations for passports for minors came into effect. The new rule extends the two–parent consent and personal appearance requirements to all minors under the age of 16. This means that 14 and 15 year-olds must also now comply with the same rules that previously applied to minors under 14. Most significantly, a minor under age 16 MUST appear with BOTH parents when submitting an application for a passport. The appearance of both parents cannot be waived, (although there is provision, in certain circumstances, for one parent to present a letter of authorization, together with one attending parent). In addition, the application MUST include the following items:
- Application Form DS-11
- Proof of U.S. Citizenship of the minor
- Documentation of Evidence of Child's Relationship to Parents/Guardians
- Parental Identification
- Notarized Parental Application Authorization Letter (If One Parent is absent)
- Two Passport Photos (2x2 inches)
- Fees for application (US$85)
- Social Security Number
- Child's most recent passport
Special Requirements for Children Under Age 16
All children regardless of age (including newborns and infants) must have their own passport. The child and both parents MUST be present to submit the application. (see item 5. below if one or both parents are not able to be present)
1. Fill out Application Form DS-11 - Please do not sign the DS-11 application form until the consular officer instructs you to do so.
2. Submit Proof of U.S. Citizenship (for minors under age 16)
Minors under age 16 must establish proof of citizenship AND proof of relationship. Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s). See number 3 below for proof of relationship.
All documents submitted with the application as evidence of U.S. citizenship will be returned after verification by the consular officer.
For proof of citizenship, you will need to submit one of the following:
- Certified U.S. birth certificate; or
- Previous fully valid U.S. Passport; or
- Report of Birth Abroad (Form FS-240); or
- Certification of Birth Abroad (Form DS-1350); or
- Certificate of Citizenship or Naturalization from USCIS.
Note: A certified birth certificate has a registrar's raised, embossed, Impressed or multicolored seal, registrar's signature, and the Date the certificate was filed with the registrar's office, which must be within 1 year of your birth
A Delayed Birth Certificate filed more than one year after your birth may be acceptable if it:
- Listed the documentation used to create it and
- Signed by the attending physician or midwife, or, lists an affidavit signed by the parents, or shows early public records.
Note: The following are not proof of citizenship
- Voter registration cards
- Army discharge papers
3. Present Evidence of Child's Relationship to Parents/Guardians (for minors under age 16)
Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s) / guardian(s).
You will need to submit one of the following:
- Certified U.S. birth certificate (with parents' names); or
- Certified Foreign Birth Certificate (with parents' names and translation, if necessary); or
- Report of Birth Abroad (Form FS-240) (with parents' names); or
- Certification of Birth Abroad ( Form DS-1350) (with parents' names); or
- Adoption Decree ( with adopting parents' names); or
- Court Order Establishing Custody; or
- Court Order Establishing Guardianship.
Note: If the parent(s)'/guardian's name(s) is/are other than that on these documents, evidence of legal name change is required.
4. Provide Parental Identification (for minors under age 16)
- Valid Drivers License
- Valid Official U.S. Military ID
- Valid U.S. Government ID
- Valid U.S. or Foreign Passport with recognizable photo
- Naturalization/Citizenship Certificate from USCIS with recognizable photo
- Alien Resident Card from USCIS
Note: Your Social Security Card does not prove your identity.
5. Present Parental Application Permission Documentation (for minors under age 16)

Both parents must appear together and sign or
- One parent appears, signs, and submits second parent's notarized statement of consent authorizing passport issuance for the child (a notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16, or a notarized written statement with the same information on a sheet of paper from the non-appearing parent) or
- One parents appears, signs, and submits primary evidence of sole authority to apply (such as one of the following):
- Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent; or
- Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent; or
- Court order granting sole custody to the applying parent (unless child's travel is restricted by that order); or
- Adoption decree (if applying parents is sole adopting parent); or
- Court order specifically permitting applying parent's or guardian's travel with the child; or
- Judicial declaration of incompetence of non-applying parent; or
- Death certificate of non-applying parent.
If none of the above documentation is available, the applying parent/guardian should submit a Form DS-3053: Statement of Consent: Issuance of a Passport to a Minor Under Age 16.
Note: A third-party in loco parentis applying on behalf of a minor under the age of 16 must submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport. When the statement or affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.
6. Provide Two Passport Photos
Your photographs must be:
- 2x2 inches in size
- Identical
- Taken within the past 6 months, showing current appearance
- Color
- Full face, front view with a plain white or off-white background
- Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
- Taken in normal street attire
- Uniforms should not be worn in photographs
- Do not wear a hat or headgear that obscures the hair or hairline unless worn daily for religious purposes.
- If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture.
- Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons. A medical certificate may be required.
There is a self-operated photo booth on the embassy premises that can generate two pieces of U.S. passport photos for S$10.00. The machine is located on the ground floor at the visa applicants’ waiting area. Please be sure to have the exact change as the machine can only accept a Singapore ten dollar note and the cashiers cannot provide change.

7. Fees and method of payment
Fees may be paid by credit cards (VISA, MasterCard, American Express, Discover), U.S. dollars or Singapore dollars equivalent. Click here to see passport fees.
8. Provide a Social Security Number
If you do not provide your Social Security Number, the Internal Revenue Service may impose a $500 penalty. If you have any questions please call your nearest IRS office.
