Visa pages can be added to a US passport by going through the queue and submitting a completed DS-4085 visa pages application form. This transaction is no longer available on the spot. Passport collection is available the following business day between 3:00-3:30pm in the Visa Services waiting room. The fee for this service is no longer free but US$82. Fees may be paid by credit cards (VISA, MasterCard, American Express, Discover), U.S. dollars or Singapore dollars equivalent.
The U.S. Embassy may add a maximum of three visa page sets to passports that remain in good condition. If your passport is eligible, we will be happy to add two visa page sets for the price of one (US$82). If you find that you have run out of pages and your passport is in worn condition, please permit 2 weeks to apply for a replacement passport. This is a reminder that you can travel on your current passport while the new one is being made.
If you appoint a representative to bring your passport to the Embassy, please make sure the representative brings a signed authorization letter from you, a valid photo I.D, and the signed application (DS–4085).
Note: When applying for visa pages for a child who is under 16, at least one parent MUST sign the application. It is not required that the child appear in person. However, the officer reserves the right to request further documentation if there are concerns about the identity of the child or the relationship between the parents.
