PASSPORT APPLICANTS AGE 16 AND ABOVE
The passport application: Complete the appropriate form.
- Form DS-82: Use this form if you are renewing a full validity passport that was issued less than 15 years ago AND can be submitted.
- Form DS-11: Use this form if you are a first time applicant, or your previous passport was issued over 15 years ago.
Proof of U.S. citizenship:
- Previous full validity U.S. passport
- Certified Birth Record
- Consular Report of Birth
- Naturalization Certificate or
- Certificate of Citizenship
The citizenship evidence submitted for minors under the age of 14 must list both parents' names. Citizenship documents presented should be originals or copies certified by the issuing U.S. authorities. Photocopies or copies certified by commercial entities are not acceptable.
Proof of Identity:
You may submit one or more of the following items containing your personal information with your signature and a recent photograph that clearly bears an identifiable resemblance of you:
- U.S. or foreign passport;
- Valid U. S. driver’s license (not temporary or learner’s license);
- U.S. government (Federal, State, municipal) identification card or pass. Temporary, altered, or commercially issued documents are not acceptable.
U.S. Passport photos:
Submit two photos that are identical and in color, taken within the last 6 months. Photo size must be 2" by 2" on photographic paper with a light or white background. The photos must be clear and showing the front view, full face, taken in normal civilian attire without a hat or dark glasses. Retouched photos, snapshots, most computer generated prints and magazine photographs are unacceptable.
Fees:
- US$100: First time applicants above age 16
- US$75: Renewal of adult passports
We accept most major international credit cards, U.S. dollars, U.S. equivalent in Singapore dollars or U.S. travelers’ checks. Personal checks, debit cards or payment by NETS are not acceptable.
Personal Appearance:
The following applicants must appear in person to submit the passport application and take an oath administered by the consular officer.
- First time applicant.
- Applicant whose previous passport was lost, stolen or damaged.
- Applicant whose previous passport has expired and it was issued more than 15 years ago.
- Applicant whose previous passport has expired and it was issued the when he / she was under age 16.
- Applicant whose name has changed since the issuance of the previous passport and there is no legal document formally recording the name change.
